I've applied many of those practices and patterns listed above as I encountered them over a 15-year journey as a technology leader. I learnt to apply SCRUM and XP in a government department, Kanban and FDD in science technology, Portfolio management, Cynefin & Servant leadership in a Bank, Scaling Agile, Agile HR, Sociology and Psychology in eCommerce and the importance of Leadership throughout.

Through this process, I've documented my experience with these practices and patterns and have tried to combine the myriad of organisational dynamics into a unified theory of sorts. Leadership is a such key element in the success of any organisation, so I have focussed on identifying key enterprise practices I feel we as leaders and change agents in organisations should have on their priority list. I want to discuss the key dysfunctions we are still seeing in almost every organisation. I consider what we should do and why we haven't. I call on 15 years of experience as a leader attempting to get better at avoiding these pitfalls and consider the underlying and systemic causes that have made me fail many times. I hope to discuss with the audience full of smart brains ... Why are we not there yet? What is the next step?
